At BHDG we care deeply about your oral health and its connection to your overall well being. We understand and respect your decision not to have your complete oral examination done today. We want to ensure that you are aware of the risks of delaying treatment and would like to educate you as to why this is a vital part of your dental visit. A COE involves a complete assessment of the head & neck area. It is an evaluation and recording of all extra oral, intraoral, soft and hard tissues. Without this, pertinent information may be missed, and further complications may arise. I understand that by not having the recommended treatment conditions may currently exist that will be undiagnosed, and/or may arise at any time in the future that could have been prevented and detected earlier. I have had an opportunity to ask questions about COE and the risks involved with not proceeding with this examination. I am refusing to have this exam completed at this time.
